Implant Age Is Not the Only Factor
There is no universal date when every breast implant must be removed or exchanged. Some patients need another operation because of a complication, while others pursue surgery because their body or aesthetic preferences have changed.
Regular follow-up remains important even when your breasts look and feel normal. Silicone implant ruptures can occur without obvious symptoms. For patients without symptoms, current FDA breast implant guidance recommends an ultrasound or MRI five to six years after silicone implant placement and every two to three years after that. Your surgeon may recommend a different schedule based on your implant type, medical history, or symptoms.
Saline implant deflation is often easier to detect because the affected breast may noticeably lose volume.
Signs That Should Be Examined
Rather than waiting for a particular implant anniversary, contact a plastic surgeon when you notice a new or persistent concern. Reasons to arrange an examination include:

- Increasing firmness or hardness
- A visible change in breast size, shape, or position
- New asymmetry
- Rippling or implant displacement

- Suspected rupture or saline deflation
Hardness, pain, and distortion may be signs of capsular contracture. This condition develops when the scar capsule surrounding an implant tightens. Treatment depends on the degree of contracture, the implant’s condition, and the surrounding breast tissue.
Your Body and Preferences May Change
Implant exchange is not limited to treating complications. Pregnancy, breastfeeding, weight fluctuations, aging, and gravity can alter the skin and natural breast tissue around an intact implant. You may also decide that you prefer a different size, shape, or profile than you chose during your original surgery.
When sagging is present, replacing the implant alone may not raise the breast or remove loose skin. A breast lift with implants can address both breast position and volume when appropriate.
