Types of Breast Implants

Miami cosmetic surgeon Dr. Gershenbaum routinely performs breast augmentation using either saline filled breast implants or the new FDA approved cohesive gel silicone implants. Most plastic surgeons and patients agree that the new cohesive silicone gel implants have a more natural feel. They are also less likely to produce rippling than saline breast implants, although saline implant are also available upon request. The cohesive gel implant will generally require a slightly larger incision to allow placement of the implant compared to a saline breast implant, however, Dr. Gershenbaum uses a specialized funnel that allows placement of a larger silicone implant through the standard smaller incision.

Breast Implant Location

The breast implant may be placed above the chest muscle (pectoralis muscle) or under the chest muscle. Dr. Gershenbaum generally places the implant under the chest muscle. Reasons for submuscular placement include a lower rate of capsular contracture (firmness and hardening of the healing tissues around the implant) and a more natural result due to more soft tissue coverage over the implant. Placement of the implant beneath the muscle may produce some movement and/or distortion of the breast and implant with active movement or exercise of the chest muscle, arms and shoulders.

Breast Implant Location

Breast Implants also come in different shapes and profiles. Anatomic or “teardrop” shaped implants were created with a more natural shape in mind and designed to be fuller at the lower half similar to a natural breast. However, this distinct vertical orientation can result in distortion or breast asymmetry if the implant should rotate. A textured surface allowing tissue in-growth helps reduce the chance of rotation however may result in more visible or palpable rippling. For these reasons, anatomic implants are not commonly used.

Dr. Gershenbaum routinely uses the most common smooth, round implant. Since the breast implant is smooth and round, it is free to rotate without any adverse consequences. With gravity alone the cohesive silicone gel or saline will gravitate or fill the bottom portion of the implant when sitting or standing erect or fill the breast more uniformly when lying down supine, more closely imitating a natural breast shape. Textured implants allow for in-growth of the tissue during the healing process creating a velcro like effect between the implant and natural tissue. This adherence to the overlying tissue may create a more palpable implant and / or more visable rippling especially in thin patients. Unless otherwise indicated, Dr. Gershenbaum will use smooth round implants.

High Profile implants are also available with a more narrow base diameter (width) and an increased projection as compared to the standard Midrange Profile implants. High Profile implants are generally used when more projection is requested without the concomitant increase in base diameter (width). Dr. Gershenbaum routinely uses both Midrange Profile and High Profile Implants depending on the patients exams and desired goals.

Size of Breast Implants

Choosing the right size implant is also very important in getting the desired result. Dr. Gershenbaum will measure the width of the breast as well as the distance from the nipple to the inframammary crease, and take note of the thickness of the skin and subcutaneous tissue. He will then suggest a range a sizes that will fit properly and produce a natural result. The patient has the ultimate decision and, at times, will have to decide on how large an implant verses how natural they would like to be.

Size of Implants

Using too large an implant for the space that is available may take away from a natural look, especially true with a thin patient and a small breast or when a patient has an overly short distance between the nipple and the inframammary crease. Dr. Gershenbaum may recommend a High Profile implant for those who want to appear larger, with more projection but need to limit how far the implant extends laterally on the side of the breast, and / or inferiorly to the inframammary crease. A thorough exam is needed to determine which size and profile implant will best match the patient with their desired goals.

Breast Augmentation Surgery in Miami with Dr. G

An augmentation mammoplasty is the medical term for a breast augmentation. A breast augmentation is a surgical procedure that uses breast implants as a means to increase the size of the breasts. Many women choose to have a breast augmentation to restore volume to their breasts following significant weight loss and/or pregnancy. An augmentation can also be used to improve the symmetry of uneven breasts. In addition, breast implants are frequently used to reconstruct the breasts following a mastectomy or injury.

Types of Breast Implants Available in Miami

There are two types of breast implants that Dr. Sam Gershenbaum uses in his practice in Miami; saline and form-stable silicone (aka Gummy Bear Implants). Dr. Sam Gershenbaum assists his patients in determining which type of breast implant will provide the optimal results.

Saline Implants

A saline implant consists of an outer silicone shell and sterile saltwater (i.e., saline).

Advantages of saline breast implants:

  • Implants are filled after they are inserted; therefore, adjusting the size is simple.
  • Saline is simply sterile saltwater; consequently, it is non-toxic to the body. In the event of an implant leak, health concerns are minimal.

When compared to silicone implants, saline implants are a more economical option.

Gummy Bear Breast Implants

Gummy bear implants consist of a silicone outer shell filled with a form-stable silicone. Form-stable silicone implants have a consistency resembling that of a gummy bear, which has earned them their nickname: A gummy bear implant can be cut in half and the gel inside remains intact.

Advantages of gummy bear implants:

  • These implants are available in an assortment of shapes and sizes.
  • Since the form-stable silicone holds its form, folding or rippling is of little concern.

Breast Implant Placement Options

There are two techniques utilized when placing breast implants:

A submammary/subglandular placement involves inserting the implant directly behind the breast tissue or into a pocket above the pectoral muscle, when an implant is placed submuscularly, it sits beneath the pectoral muscle itself. In general, Dr. Gershenbaum prefers to place breast implants beneath the pectoral muscle because there is less chance that the patient will experience capsular contracture (a hardening of the tissues that surround the implant. In addition, Dr. Gershenbaum prefers the more natural-looking results he can accomplish with the submuscular implant placement.

Choosing the ideal placement for an individual’s breast implants requires that Dr. Gershenbaum considers:

  • The patient’s physical characteristics.
  • The size and type of implant she desires.
  • The movement of her pectoral muscles.

Implant Insertion Choices

Just as with breast implant placement options, implant insertion choices depend on a variety of factors, including:

  • The type of implant being used.
  • Desired degree of breast enlargement.
  • The patient’s anatomy.

Although Dr. Gershenbaum prefers to use the areolar incision technique, he does offer his patients all of the available insertion options, which include:

Inframammary

Dr. Gershenbaum creates an incision at the base of the patient’s breast (within or near the breast crease).

Axillary

An incision is created within the natural crease of the patient’s armpit.

Trans-Umbilical (Specifically Designed for Use with Saline Breast Implants)

Dr. Gershenbaum creates an incision in the upper part of the patient’s navel.

Areolar – This is Dr. Gershenbaum’s Preferred Insertion Technique

An incision is made within the border of the lower portion of the areola, which is the dark skin that surrounds the nipple.

Dr. Gershenbaum prefers the areolar insertion technique because, in the event of scar formation, the patient’s scar remains well-hidden within the pigmented area of skin surrounding her nipple. Furthermore, this incision allows for a patient to receive a breast lift in conjunction with a breast augmentation.

Breast Augmentation Surgery in Miami – The Procedure

A breast augmentation procedure is generally an outpatient procedure; therefore, since anesthesia will be administered, patients are required to bring a licensed driver with them on the day of their surgery.

1. Anesthesia is Administered

As previously mentioned, patients do receive medicines to ensure they remain comfortable throughout their breast implant surgery. A licensed anesthesiologist administers these medications: The medications used during a breast augmentation are IV sedation or general anesthesia.

2. The Incisions Are Created

Dr. Gershenbaum and the patient will have previously discussed the desired location of her incisions. All incisions are made in such a way as to provide the patient with the least amount of scarring possible. Additionally, due to proper incision placement, any scars that remain will be well-hidden within the natural creases or pigmented skin of the patient’s body.

3. Insertion and Placement of the Breast Implants

Once the incision has been made, Dr. Gershenbaum will gently insert and position the patient’s breast implant.

4. Closing the Incisions

Once the implants are in place and Dr. Gershenbaum is satisfied with their location, size, and appearance, he closes the incisions.

Dr. Gershenbaum uses a layered suture technique within the breast tissue. He uses sutures, skin adhesive and/or surgical tape to bring the outer skin incision together. Although the incision lines will be visible directly following the breast augmentation procedure, as time passes, these lines will fade.

5. Placement of Bandages and Support Bra

Immediately after the breast augmentation is complete, patients’ breasts are bandaged and a support bra is placed over them.

6. Recovery

Unless Dr. Gershenbaum decides he would like to keep the patient overnight, the patient will be released to her driver within a few hours of the completion of her breast augmentation procedure.
